Export declaration checklist  A checklist for export goods that require formal customs clearance.
Power of Attorney  The letter of authorization (POA) must be provided for each shipment and must be stamped with your company.

US Customs Clearance Common Documents

Artwork Declaration  This is a statement that the art object is an original work, and full name of the artist and date of production must accompany the documentation. Original works of art may enter the US duty free.
Shipper's Declaration of Repair  This declaration is required for customs to verify the kinds of repair/alternation and original export; duty tax will be accessed on the value of the repair or alternation fee.
Capacitor Work Sheet  This checklist is needed to assist with the import clearance of capacitor into the United States.
Bearings Certification  This checklist is needed to assist with the import clearance of ball bearings and roller bearings into the United States.
Fabric Detail Sheet  This checklist is needed to assist with the import clearance of textile shipment into the United States.

HS-7 Motor Vehicle Safety Form  This form is required to be filed for any motor vehicle or article of a motor vehicle that is subject to DOT(US Department of Transportation) safety standards.
Drop Ball Test Certification  This form is used to advise the eyewear (specifically the lenses used in the sunglasses) are impact resistant and have been submitted to and have passed the FDA required drop ball test. It is required by the FDA and US Customs.
Radiation Declaration - Radiation Form 2877  This form is required by the US Food and Drug Administration for all items that emit/transfer radiation.
