FedEx Ground Bar-Code Label (U.S.)

Every FedEx Ground package sent within the U.S. needs a bar-code label and an address label. This can be:

  • A combined bar code and address label produced by FedEx electronic shipping tools or third-party parcel-processing equipment; or
  • A preprinted bar-code label plus an address label you create yourself. These bar-code labels contain your company's shipper number and a unique tracking number for routing your package.

You can order FedEx Ground bar-code labels by calling 1.800.GoFedEx (1.800.463.3339). Alternatively, FedEx Ship Manager® at fedex.com can help you generate labels electronically right from your desktop. Remember, there is a different bar-code label for every service or service option you choose. If you don't have your own bar-code labels, you can also take your U.S. ground packages to most FedEx drop-off locations and we can process them for you.

Completing the FedEx Ground Pick-Up Record

There's no need to complete a FedEx Ground Pick-Up Record manually if you prepare your FedEx Ground bar code/address labels using FedEx Ship Manager at fedex.com or our other electronic shipping tools, which automatically transmit your package information to us and generate a manifest. If you prefer using manual preprinted bar-code labels, complete this sheet and send it along with each day's shipments.

  • Enter pickup date and shipper address.
  • Enter your FedEx account number. List the total packages and bar-code labels for that day, and any that were damaged and voided.
  • Unless you are using extra services or service options (see below), you are ready to send this with your shipment.

Indicating Extra Services on the FedEx Ground Pick-Up Record

  1. Complete the Extra Services section when you select a service option for your shipment, or have packages that fit the categories listed below. For these packages, peel one of the 15-digit tracking ID numbers off the bar-code label and place it on the pick-up record in the column labeled Tracking ID.
  2. If shipping C.O.D., list the amount to be collected in the currency of the destination country — either U.S. or Canadian dollars.
  3. Use the declared value area when the value of your package exceeds $100. The maximum declared value is $50,000 per package. (The C.O.D. amount is not a declaration of value.)
  4. If you have packages less than 30 lbs., greater than 84" but equal to or less than 108" in length plus girth, mark the appropriate Oversize column. These will be billed at the 30-lb. rate.
  5. If you have packages less than 50 lbs., greater than 108" but equal to or less than 130" in length plus girth, mark the appropriate Oversize column. These will be billed at the 50-lb. rate.
  6. If you have packages 150 lbs. or less and greater than 130" in length plus girth, mark the appropriate Oversize column. These packages will be billed based on the package's actual rounded weight or at the 90-lb. rate, whichever is greater.
  7. Use the Additional Handling box to record any package exceeding 60" in length, any article not enclosed in an outside shipping container, any article enclosed in a shipping container made of metal or wood, and drums and pails less than 5 gallons not enclosed in a corrugated cardboard container.
  8. If you want automatic acknowledgment of delivery, mark the A.O.D. box and affix an A.O.D. card to the outside shipping container.
  9. If you have a package going to a residential address, mark the RES box. An additional handling fee will apply, and no money-back guarantee will be available. (For ground residential delivery shipments with a money-back guarantee, use FedEx Home Delivery®.)
  10. List the number of extra-services packages you have, and complete the box at the lower right of the pick-up record.
